The federal government is urging parents and caregivers to stop using a popular children’s slide playset immediately after one child died and others remain at risk of a deadly design flaw.
The U.S. Consumer Product Safety Commission issued the warning Thursday for the Model WPX-4 Slide Playset, made by Wenzhou Yidian Amusement Equipment Co., Ltd. The agency said the playsets have multiple openings large enough for a child’s torso to pass through — but not their head — creating a head and neck entrapment hazard.
The agency confirmed one death linked to the product: a 17-month-old child who became entrapped in the playset.
What to look for — and what to do
The plastic playsets are designed for indoor or outdoor use and come with several activity features, including a tunnel, a slide, and a basketball hoop. Some versions include a vehicle-shaped component on the side. The sets are sold in red, navy, coffee, gray, green, and yellow.
The model number WPX-4 and the manufacturer’s name appear on a label on the product.
The CPSC says caregivers should:
- Stop using the playset immediately
- Throw it away — do not sell it, donate it, or give it away
- Report any incidents at SaferProducts.gov or by calling 800-638-2772
Manufacturer refuses to cooperate
The situation is complicated by the fact that Wenzhou Yidian Amusement Equipment Co., Ltd. has not agreed to an acceptable recall of the product. Because of that, the CPSC is not offering a refund or replacement — it is simply urging consumers to dispose of the playset entirely.
The warning was issued Aug. 13, 2026, and assigned product safety warning number 26-703.
